Young People’s Substance Misuse Worker

Job Introduction

HumanKind is a partner of Staffordshire County Council and they currently have an opportunity for you to join their young peoples team in Staffordshire working alongside our Future Matters Intensive Prevention Service. This is a rewarding position, working alongside families with children and young people that are at risk of becoming edge of care.

Job Purpose
This role will support the overall service delivery of young people involved in the Intensive Prevention Service (IPS) where they are at risk of becoming edge of care; and for families accessing Breathing Space, to receive support around substance misuse. 

Main Responsibility

In this role you'll:

  • Undertake assessments, develop care plans and manage regular reviews in order achieve agreed goals for young people using the service
  • Deliver interventions to address drug and alcohol use and associated issues, and reduce substance related harm to the individual and the wider community
  • Provide harm minimisation advice and information to children, young people and families
  • Work with children and young people who require support as part of a reunification plan from local authority care back home
  • To deliver innovative group work sessions to groups of young people, families, practitioners and staff
  • To deliver interventions from multiple venues across Staffordshire.
  • Promote the service through a variety of means and resources to individuals, communities and professionals within Staffordshire.
  • Embed provision and service within targeted communities of Staffordshire
  • To liaise and network with a wide range of professionals working with young people across Staffordshire, maintaining effective partnerships and working agreements to improve the outcomes for the individual
