Social Care Assessor

Job Introduction

The First Contact Team, engages with residents of Staffordshire, carrying out strengths based, outcome focused assessments via the telephone and other virtual means. 

We are looking for a Social Care Assessor who shares our values and commitment and are passionate about improving the lives of our residents. 

Main Responsibility

You will play a key role in assessing and reviewing the social care needs of adults with a focus to provide a holistic, personalised service to residents over the age of 18 who have physical disabilities and older people with mental health disabilities.

We would like to hear from people who are passionate about delivering the best services to our service users.

Your duties will include:

  • undertaking assessments of individual and carers needs and risks to encourage independence, provide support and prevent harm.
  • working in partnership with adults service users and carers using a person-centred approach, with the aim of maximising independence, wellbeing and choice.

The Ideal Candidate

You will have social care experience together with analytical assessment skills and be able to identify, assess and manage risks.
