Social Care Assessor - Assistive Technology Specialist

Job Introduction

The First Contact Team, engages with citizens of Staffordshire, carrying out strengths based, outcome focused assessments via the telephone and some in person assessment work across the county for carers and young carers. The Care Act 2014 compliant assessments determine citizens assessed eligible needs while promoting their assets and facilitating their independence.

Adult Social Care support people to live their lives to the fullest. Technology Enabled Care (TEC) is a vital area of growth and investment for Adult Social Care and a key enabler of the ASC vision and ‘Early Help Offer’. It has been identified as a catalyst to prevent escalating needs, maximise independence and improve quality of life.

The Social Care Assessor- Assistive Technology Specialist will build and enhance the use of assistive technology and digital solutions across the ASC workforce and beyond, developing networks. This will include influencing practice, promoting the assistive technology and digital solution offer within local communities, and building confidence in the ASC workforce.

The Social Care Assessor- Assistive Technology Specialist will work collaboratively with partners to deliver one operating model, providing a conduit between the providers and the ASC Workforce, and ensuring that the potential for technology is maximised for Staffordshire citizens.

The team is part of the wider Adult Social Care Team within the council. All that we do focuses on how we will develop the right culture, support and skills to keep making a difference for Staffordshire’s communities.

The role of the assistive technology specialist will work alongside social care practitioners and social workers across adult social care in order to complete a Digital Assessment of the person’s home and resources available e.g. smartphone/tablet/Alexa /Echo/Gennie.

Main Responsibility

2. You will inform the assessment of person’s current level of skill and confidence.

3.  Provide training and support – 1:1 and possible groups face to face or using Zoom/Teams etc.

4. Follow up after initial assessment to ensure skills are retained/developed to inform the review process.

5. Understanding of and registration with NHS App and other apps – mindfulness/proactive prevention, fall – social isolation/ carers. Push notifications re NHS App if available.

6. Analyse information to understand risks and develop strategies to explain, avoid or manage risk appropriately.

7. Negotiate and agree appropriate, tangible and realistic outcomes with individuals and families.

8.  Collaborate with other professionals, within Adult Social Care or other organisations, in order to fully understand need and enable individuals to achieve their outcomes and to be as independent as possible.

The Ideal Candidate

1. Have knowledge and understanding of assistive technology, how this can be utilised and how to set it up. 

2. Have experience in assessing, risk assessing and support planning.

3. To be able to share knowledge and learning with others in regards to assistive technology.
