We are looking for motivated people who are passionate about providing outstanding care quality for adults with care and support needs. We have an exciting opportunity to join a brand-new service supporting people in their homes to regain their independence and learn new skills.

Our Independence at Home service will prevent hospital admissions and support people to age in place and stay at home for as long as possible.

This role requires travelling within the community, mainly in the north of the county, so a full UK driving licence and access to your own vehicle is essential.

We have full time position available with a range of shifts between 7am and 10pm, which will include weekend and bank holiday working. We require our team to work flexibly, which means you may be required to work days, evenings, and split shifts.

Post is subject to an enhanced DBS check.

  • We provide full paid training and ongoing development to enable you to pursue your career aspirations
  • BLOCK pay, meaning you will be paid for the whole shift duration, including travel time
  • For travel, we will reimburse you at £.45/mile, paid from a council base nearest to your home
  • Minimum shift is 4 hours in duration
  • Supervision and support to enable you to deliver excellent care

Main Responsibility

Working flexibly across various locations in north Staffordshire, you will play an important role providing an excellent level of care to enable the people we support to have a comfortable and dignified life within their own homes. As a Senior Support Facilitator, you will:

  • Assess the home environment, risks, and develop care plans
  • Complete regular reviews with people, their relatives, and other professionals
  • Administer medication
  • Using an active support approach, facilitate people to regain their independence and learn new skills
  • Support people with a range of needs, including personal care and eating and drinking
  • Maintain professional relationships between other health and social care professionals
  • Guide and coach Support Facilitators in the delivery of care plans

The Ideal Candidate

What you will bring

  • enthusiastic about working with people and are caring, sensitive and patient while supporting people to be as independent as possible.
  • good communication and interpersonal skills and a positive outlook on life.
  • can adapt to reflect the changing needs of the people in their own homes and can remain calm in challenging situations.
  • able to adapt and respond positively to change and prioritise tasks and work within time restraints

What you will have

  • NVQ Level 2, or Care Certificate in a relevant discipline(s)
  • Driver’s licence and access to a vehicle
  • Experience of working in a team
  • Experienced with supporting people through either work or life experience.
  • Experience with administrating medication
  • A basic understanding of the legislative and regulatory framework affecting vulnerable adults, including The Care Act (2014) Safeguarding, Liberty Protection Safeguards, Mental Capacity Act, and the regulatory requirements as applicable to the service.
  • Knowledge of risk management and positive risk taking
  • Knowledge of the principles of person-centred support, reablement, active support model and strengths-based approaches
