Job Introduction
This is an exciting opportunity to help us shape how we can best work alongside our SEND (Special Educational Needs and Disabilities) and school/colleges.
As a newly established role, we will be learning from the people we work with as the role develops, ensuring we help young people reach their goals and aspirations. The purpose for this role is to attend and actively participate in EHCP's (Education Health and Care Plan) ensuring the right advice and guidance is given for those that need or likely to need adult social care.
The role will be the link for our SEND, school and colleges who need advice and guidance developing their own networks within these organisations.
Main Responsibility
As an Education Liaison Assessor, you will work alongside SEND keyworkers, Schools and colleges to participate and support the EHCP process. You will have a strength based approach in ensuring we help provide the right support and guidance for young people to reach their goals.
you will also:
- work with young people and their parents/carers throughout the EHCP process
- undertake flexible and imaginative approaches in offering support to adults, their families and carers.
- promote good outcomes for the person.
- Support in good networking with schools and colleges
- Be the link for adult social care within those EHCP's
- Be the link for schools and colleges for adult social care.
The Ideal Candidate
You'll have:
- experience of working with individuals and understand the education health care plan process
- motivation to support young people and their families through difficult transition
- great networking skills
- ability to support people to reach their goals
- experience of being a team player, you’ll be able to flex to the needs of the team
- a proven track record of a person centred, and asset focused outlook and you’ll have high professional standards.
