Job Introduction
We are looking to recruit two Advanced Practitioners to work within the First Contact Adult Social Care and All Age Carers Team.
This key role will take responsibility for the working across Older People, Physical and Learning Disabilities, Hospital Discharge referrals via Home First and all age Carers assessments, as such we are seeking experienced Advanced Practitioners with a range of knowledge and skills across this broad agenda.

Main Responsibility
Key responsibilities will include:
- supervising and supporting the professional development of social workers and social care assessors.
- managing unpredictable daily changes to work pattern.
- using initiative to organise daily duties whilst adhering to SCC policy and procedure to meet service demands.
- be responsible for the quality assurance of statutory documents and promoting and embed the ethos of “Just enough Support”.
You will also:
- contribute to the development of best practice within the service.
- ensure flexibility and management of a healthy work life balance.
- ensure staff are motivated to successfully deliver objectives around adult engagement.
- promote and support the identification of actual or potential safeguarding issues and ensuring that these are promptly acted upon in line with policy, procedure and legislative guidance.
- co-work cases with less experienced or non-social work qualified members of the team and taking responsibility as part of that process for sharing and developing their learning.
Take responsibility for the provision, of:
- assessments under the Care Act inclusive of all statutory functions
- participation on the inter-agency best interest assessor rota for adults
- participation on the inter-agency AMHP rota
The Ideal Candidate
You will have:
- demonstrable management experience.
- knowledge and experience of working within Adult Social Care.
- knowledge and experience of applying the Mental Capacity Act assessments, Best Interest Assessments/DOLs.
- knowledge of mental health legislation including section 117.
- understanding of the principles of the different funding streams available to support adults.
- experience of working as part of a team, in developing the professional practice of colleagues.
We require you to be flexible to work at any other location across the service, as and when required and be available to work out of hours, evenings and weekends when required.
